The Membership Plan is NOT dental insurance and CANNOT be used in conjunction with ANY dental insurance plan or with ANY other discounts, including all 3rd party financing. There are no refunds with the plan. The plan runs for one year from the date you sign up. Full payment of membership required. These benefits are ONLY for the plan period of 12 months on effective date. Cannot be utilized in hospital care settings. Does not include sedation fees. Not used in conjunction with dental care covered under automobile or accidental insurance coverage, or injuries covered under workmen’s compensation policies.
